About the role

The Care Coordinator will provide non-clinical care and practical psychosocial support to service users, families, and carers, helping clients navigate services, maintain continuity of care, and achieve their recovery goals. Operating within the Multidisciplinary Service Team (MDT) funded by the Western Victoria Primary Health Network (WVPHN), the role will support communities across Warrnambool, Camperdown, Portland, and Hamilton.

We are seeking a passionate, self-motivated Care Coordinator for a fixed term, part time contract (26.6 hours per week) opportunity to work with a dynamic team in the Warrnambool area. You will be responsible for providing practical psychosocial support and care coordination, helping clients recover, navigate services, strengthen community connections, and live the life they choose.

 

Key responsibilities

  • Provide recovery-focused, non-clinical care to clients following a suicide attempt or crisis.
  • Support clients to navigate community, clinical, and social services.
  • Maintain continuity of care and coordinate with multidisciplinary teams.
  • Facilitate referrals and provide practical psychosocial support tailored to individual needs.
  • Support families and carers through education, guidance, and advocacy.
  • Build relationships with local service providers, healthcare partners, government agencies, and community organisations.
  • Actively participate in team meetings, case reviews, and reflective practice.
  • Travel across Warrnambool, Camperdown, Portland, and Hamilton as required.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy, or other health-related field.
  • Experience working with people with mental health issues, suicidal ideations and attempt, complex needs and with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers and key stakeholders.
  • Proven capability to work autonomously and as part of a person-centred team.
