About the role
The Sub-Acute short stay ‘step up, step down’ Youth Prevention and Recovery Care (YPARC) service, delivered in partnership with Austin Health and Mind Australia, supports young people with psychosocial disability to transition from an acute hospital setting (Step-down) or to prevent hospital admissions by providing extra support to those entering directly from the community (step-up). The Youth Prevention and Recovery Care (YPARC) service is a residential program providing short-term, intensive treatment for up to 28 days to young people aged between 16-25 years living with mental health challenges.
We are seeking two compassionate, self-motivated Community Mental Health Practitioners for 2x Permanent, Part Time (30.4 hours per week) opportunities to work alongside a caring team in the Heidelberg area. With 24/7 rotating roster + active night shifts, the Part-Time renumeration + penalty rates of these positions will be comparable to Full-Time earnings! In the role you will provide intensive person-centred, recovery-oriented supports to clients to achieve their wellbeing goals, build capacity and daily living skills, strengthen social and community connections, and live independently in the community.
Key responsibilities
- Plan and develop group work programs that will assist clients to build their skills, focus on their recovery and work towards transitioning back to their natural community.
- Purposefully engage with, and support, clients with actioning their recovery plan in a range of areas.
- Engage with clients to fully understand their need for assistance from local service providers with clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation and social connections.
- Support clients to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
- Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
- Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing, Healthcare or Government sectors is required.
- Demonstrated knowledge and experience in the application of theoretical approaches, practices and appropriate service responses including family inclusive practice, trauma informed theory, cognitive behavioural therapy, motivational interviewing and harm reduction therapy.
- Ability to co-design, co-produce and co-facilitate groups and education support.
