About the Service

Mind’s Outreach Psychosocial Services South Australia provides person-centred support to help people improve wellbeing, build independence, and achieve their personal goals. The service delivers support through a two-tiered model:

• Intensive Crisis Support (Tier 1): Short-term, flexible support to reduce distress, improve safety, and connect people with longer-term community services.
• Non-Acute Psychosocial Support (Tier 2): Moderate-intensity support over several months to enhance recovery, develop daily living skills, maintain housing, strengthen relationships, build social connections, and support transition to NDIS services.

About the Role

We are seeking a collaborative and empathetic Community Mental Health Practitioner for a permanent, full-time (38 hours per week) opportunity in the Port Lincoln area. You will play an integral role in working collaboratively with clients to achieve their recovery goals through one-to-one support, shared supports, workshops and group work facilitation to enhance recovery, personal growth and activities of daily living.

The role involves outreach support combined with administration duties that can be performed from home. A fleet car is provided for travel.

Key responsibilities

• Work with clients to enable them to develop independent living skills and a meaningful life by supporting them through a strength-focused, staged approach to recovery.
• Engage with clients to fully understand their need for assistance from local service providers with clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation, and social connections.
• Support families, carers, support networks and significant others to assist clients through treatment using family inclusive interventions.
• Implement a successful transition to NDIS services as a core transition pathway.
• Support clients to source, retain and maintain safe, affordable, and sustainable housing.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work or other health related field as designated by Mind.
• Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
• Awareness and understanding of the NDIS is desirable.
• Demonstrated understanding of available community services, networks and supports.
• Skills and experience in advocacy, collaboration, facilitation, evaluation, and problem solving.
