About the Role
We are seeking a compassionate, self-motivated Community Mental Health Practitioner for a permanent, part time (36 hours per week) opportunity at the Step-Up Step-Down residential service in Mackay. In this role, you will provide intensive person-centred, recovery-oriented supports to clients to achieve their wellbeing goals, build capacity and daily living skills, strengthen social connections, enhance community networks and grow confidence to live independently in the community.
About the Service
The Sub-Acute short stay Adult Step-Up Step-Down (ASUSD) residential service, delivered in partnership with Mackay Hospital and Health Service, supports adults with a psychosocial disability up to 28 days transitioning from a hospital setting back into the community or to prevent admissions whilst residing in the community. ASUSD works within a recovery focused, clinical treatment framework providing personalised one-on-one support, shared supports and group activities based on client’s strengths and goals as identified in their recovery plan.
