About the role
We are seeking a self-motivated Community Mental Health Practitioner eager to make a difference. This permanent, full-time opportunity in Ceduna allows you to support clients in achieving their recovery goals, build capacity to live safely in the community, strengthen social connections, achieve wellbeing goals and live independently.
As a Community Mental Health Practitioner, you will play a key role in empowering individuals through person-centered, recovery-oriented support. Our NAPSS ICSS Psychosocial services focus on goal setting, transition planning, as well as application and transition to NDIS services. We use a two-tiered approach:
Provide flexible, intense support and case management for a limited period (6 weeks) to reduce immediate distress, enhance personal safety, and connect clients to long-term community services.
Offer moderate support over a four-month period (extendable to 12 months) to help clients achieve recovery goals, build daily living skills, maintain housing, and foster social connections. Assist clients with NDIS eligibility, application, and transition.
