About the role

We are seeking a compassionate and self-motivated Community Mental Health Practitioner who is passionate about making a positive impact in the lives of people experiencing mental health challenges. This fixed-term, full-time outreach role offers the opportunity to support clients across Port Lincoln, the Eyre Peninsula and Far North regions, providing person-centred support in homes and community settings to help people achieve their recovery goals and build independence.

In this role, you will help people build confidence and skills for daily living, strengthen community connections, access supports and services, and improve their overall wellbeing.

This role is ideal for someone who enjoys working in the community. With no fixed office location, you will have the flexibility to complete administrative tasks from home when not supporting clients in the community. A fleet vehicle is provided to support travel throughout the region.

Key responsibilities

• Support clients to build independent living skills and lead meaningful lives using a strengths-based, recovery-oriented approach.
• Engage with clients to identify needs and connect them with appropriate services, including mental health, physical health, housing, education, and employment.
• Work collaboratively with families, carers, and support networks using family-inclusive practices.
• Support clients in transitioning to NDIS and navigating available supports.
• Assist clients to secure and maintain safe, stable, and sustainable housing.

About the Service

Mind Australia’s Outreach Psychosocial Services South Australia (OPSSA) provide person-centred psychosocial support to help people experiencing mental health challenges live independently and well in the community. The service supports people to achieve their recovery goals, build daily living skills and community connections, and transition to longer-term supports, including NDIS services where appropriate.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work or other health related field as designated by Mind.
• Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
• Awareness and understanding of the NDIS is desirable.
• Demonstrated understanding of available community services, networks and supports.
• Skills and experience in advocacy, collaboration, facilitation, evaluation, and problem solving.
• Valid Australian Driver's licence.
