About the role

The Housing with Support residential service is a long-term integrated social housing program for people living with severe and enduring psychosocial disability. The Haven residences are staffed 7 days a week and designed like a ‘home away from home’ funded through client’s NDIS packages with practice guided by Mind’s Model of Recovery Oriented Practice, Haven service model and Dual Disability framework. The service works collaboratively with clients providing personalised 1:1 support, shared supports and group activities based on strengths and goals identified in an individual’s recovery plan to build daily living skills, strengthen relationships with families and carers, enhance social connections and community networks, and grow confidence and capacity to live safely in the community.

 

We are seeking a self-motivated, empathetic Community Mental Health Practitioner – Team Leader for a Fixed Term, Full-Time opportunity to work in South Yarra. In this position you will provide leadership and support to a dedicated team ensuring the delivery of high-quality recovery-oriented services to clients, families and carers. The Community Mental Health Practitioner - Team Leader is integral in being a practice lead to Mental Health Support Workers and Community Mental Health Practitioners providing supervision, mentoring and coaching and make day-to-day decisions to ensure the operational requirements of the service are met.

 

Key responsibilities

  • Lead a dedicated team in line with organisational quality processes and policies, agreed priorities and strategies to ensure quality client service and required targets are met.
  • Oversee written tasks including case note writing, risk assessments, safety plans, behaviour management plans and incident reports.
  • Lead, coach and model a targeted, integrated and individualised responses to clients through flexible, person-centred active support.
  • Plan, deliver and develop group work programs that will assist clients to build their skills, focus on their recovery and work towards transitioning back to their natural community.
  • Make linkages and build relationships and referral pathways to maintain or create a range of local supports for clients that facilitate them living the life of their choosing in their own community.
  • Supervise the day-to-day operations of the residential service through overseeing a range of housekeeping duties.
  • Model and lead the required work environment, culture, systems and processes necessary to embed standards, behaviours and practices consistent with the values, work health and safety, policy and legislative, and practice requirements.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
  • Awareness and understanding of the NDIS is desirable.
  • Demonstrated understanding of available community services, networks and supports.
  • Ability to co-design, co-produce and co-facilitate groups and education support.
  • A track record in successful relationship development, stakeholder management and strategic partnerships.
