About the service

The Community Mental Health and Wellbeing Service (CMHWS) is a network of hubs designed to meet the increased need for immediate mental health. The hubs act as a ‘front door’ service to the mental health system where people can access services without eligibility criteria or a referral. CMHWS support people of all ages experiencing psychological distress or mental ill health and offer a blend of face-to-face support in the community, in person, telehealth and online services. The service delivers wellbeing supports tailored to a client’s individual needs including early intervention, specialist referrals, clinical care and treatment, and support for families and carers

About the role

We are seeking a compassionate, self-motivated Community Mental Health Practitioner to work in the Phillip Island/Cowes area. Working from home & in the community, you will deliver positive, person-centred recovery-orientated supports to clients to achieve their recovery goals, promote wellbeing and live safely within the community. As this position requires travel, a fleet car is provided. This role will be a fixed term part time opportunity until 30th June 2027.

Key responsibilities

• Deliver high quality in person and outreach services and facilitate positive educational and social experiences.
• Support clients to remain in the community, promote their wellbeing and enjoy a meaningful life through a staged approach to recovery.
• Provide information and mentoring to clients to establish relationships with families and carers, create community networks and access to relevant services.
• Plan and deliver group programs that aid clients to build their skills and focus on recovery.
• Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
• Expertise in working with adults and young people with mental health issues, complex needs and interacting with families and carers.
• Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
• Relevant experience in facilitating group activities for clients experiencing mental ill health.
• Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
