About the role

The Early Intervention Psychological Support Response Service (EIPSRS) provides a person-centered approach to recovery which is tailored to meet the needs of people living with mental ill health in the community. EIPSRS provides an individualised, tiered program of recovery support of up to 12 months delivered to clients with the level of engagement determined by their needs, services include individual support, one-on-one outreach services and group work. The services are designed to enhance recovery, achieve wellbeing goals, build capacity and life skills, maintain sustainable housing, improve relationships with families and carers, develop social and community connections, and live safely and independently in the community.

 

We are seeking a compassionate, self-motivated Community Mental Health Practitioner. Working from home, you will deliver assertive person-centred outreach services to clients to achieve their recovery goals, build capacity and living skills, strengthen social and community networks, maintain housing and live independently. As this position requires travel across the Melbourne metropolitan area a fully maintained company car is provided for business use.

 

Key responsibilities

  • Work with clients to enable them to develop independent living skills and a meaningful life by supporting them through a strength-focused, staged approach to recovery using agreed practice techniques and approaches.
  • Plan and develop group work programs that will assist clients to build their skills, focus on their recovery and work towards transitioning back to their natural community.
  • Engage with clients to fully understand their need for assistance from local service providers with clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation and social connections.
  • Facilitate, as appropriate, the re-engagement and maintenance of family and carer relationships.
  • Ensure all clients have an active recovery, treatment or case coordination plan consistent with the EIPSRS Operational guidelines.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing, Healthcare or Government sectors is required.
  • Demonstrated knowledge and experience in the application of theoretical approaches, practices and appropriate service responses including family inclusive practice, trauma informed theory, cognitive behavioral therapy, motivational interviewing and harm reduction therapy.
  • Ability to co-design, co-produce and co-facilitate groups and education support.
