About the Service

The Housing with Support program is a 24/7 service designed for adults living with severe and enduring psychosocial disability and co-occurring concerns and/or dual disabilities which have impacted their capacity to live independently in the community.

 

Mind’s Haven service in Clearview provides long‑term, affordable, purpose‑built housing for people with severe psychosocial disability, offering recovery‑focused supports that build independence, connection and daily living skills within a peer‑led community.

 

About the role

We are seeking an approachable, resourceful and mentoring Lead Practitioner for a permanent part time (30 hours per week) opportunity in the Clearview area. You will play a key role in leading the day-to-day operations of the service, guiding support workers to deliver safe, high-quality and recovery-focused support. Through coaching, coordination and collaboration with families, carers and external you will help coordinate services, facilitate meaningful referrals, advocate for participant needs and drive positive outcomes.

 

Key responsibilities

  • Coach staff to problem solve, develop creative solutions and remain motivated in meeting client’s goals.
  • Maintain effective communication across the team to support smooth shift operations and continuity of care.
  • Aid in the development and implementation of group programs and activities of daily living program.
  • Identify opportunities and implement strategies to continually enhance a learning culture within teams.
  • Promote a continuous learning culture by identifying development opportunities, implementing improvement strategies, and facilitating reflective practice to support team growth and enhance service delivery.
  • Support families, carers, support networks and significant others to assist clients using family inclusive interventions

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Experience and expertise in working directly with people with psychosocial disabilities and other complex support needs and with their families and carers.
  • Awareness and understanding of the NDIS is required.
  • Proven capability to coach, mentor and develop a team to achieve best quality service outcomes including experience in the provision of professional supervision.
  • A track record in successful relationship development, stakeholder management and strategic partnerships.
