About the role

The Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have NDIS plans and/or TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model. The FFS program provides centre based and tele-health services, outreach services, support coordination, and education and vocational support. The service offers clients choice and control over services so they are able to build life skills and capacity, develop social connections and create community linkages to identify and achieve their recovery goals.

 

We are seeking an empathetic, self-motivated Mental Health Practitioner for a 12 month fixed term, part time (30 hours per week) opportunity to work alongside a dynamic team in the Melbourne metropolitan area. Working from home, you will mentor and empower clients with choice and control through person centred, recover-orientated tele-health support, outreach services and support coordination to achieve recovery goals, maximise care packages, develop social connections and build capacity to live well in the community.

 

Key responsibilities

  • Deliver high quality outreach services, case managed responses and facilitate positive educational and social experiences.
  • Complete assessments, provide support coordination services and mentoring consistent with defined goals and aspirations identified in NDIS plans.
  • Support clients with strengths based, recovery-orientated approaches to develop daily living skills, capacity and resilience.
  • Provide information and mentoring to clients to establish connections with community, families and carers, create networks and access to relevant services.
  • Collaborate with a range of services, families and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
  • Expertise in working outreach and/or support coordination with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capability to plan goal focused care and implement therapeutic, recovery-oriented frameworks and practices.
  • Able to deliver person-centred support coordination services in line with client’s recovery aspirations and needs, and experience working with NDIS and FFS models.
  • Demonstrated understanding of the Community Services sector, NDIS and ability to build strong relationships with service providers, clinical teams and key stakeholders.
