About the service

The Mental Health and Wellbeing Local services are a key recommendation from Victoria’s Royal Commission into the mental health service system’s transformation.

 

The Mental Health and Wellbeing Local will act as the front door service to the mental health system and will provide a continuum of care and support, including clinical interventions, wellbeing supports, and therapeutic support through an integrated mental health and alcohol and other drug framework approach. These services will have a ‘no wrong door’ policy, ensuring accessibility to all individuals seeking support

 

 

About the role

We are seeking two professional, self-motivated Social Workers for two Permanent Part-time opportunities (26.6 hours per week) to lead, develop and facilitate a variety of targeted group work programs in the Melton area.  You will utilise expertise to deliver meaningful evidence based, time limited therapies, in response to the needs of people accessing the Mental Health and Wellbeing Local.

 

Key responsibilities

  • Provide support plans and short-term intervention methods of demonstrated effectiveness that are person-led and evidence based to meet the model of care.
  • Work collaboratively with the multidisciplinary interprofessional team and other relevant stakeholders to promote and facilitate access to primary mental health care services including the provision of support to assist staff to recognise and respond to mental health conditions and provide effective clinical management.
  • Support family, carer and supporter practice including the re-engagement and maintenance of family, carer and/or children relationships.
  • Make linkages and build relationships and referral pathways to maintain or create a range of local supports for older people that facilitate them living the life of their choosing in their own community.
  • Develop and contribute to individual and behavioural risk management plans ensuring all appropriate contemporary evidence base theory is drawn from and implemented to ensure older people achieve outcomes enhancing quality of life.
  • Design, develop, facilitate and evaluate wellbeing/therapeutic groups. Utilise a co-design process to develop groups wherever possible.

 

What you’ll bring

  • Tertiary degree qualifications in Social Work.
  • Eligibility for membership to the Australian Associate of Social Workers (AASW)
  • Previous experience providing wellbeing supports using a recovery-oriented approach in an Allied Health clinical role.
  • Experience and expertise in working directly with people with mental health challenges, AOD challenges, dual disabilities, complex needs and with their families, carers and supporters.
  • Demonstrated knowledge and clinical experience in flexible therapeutic support services including assessment, brief intervention, treatment planning and evaluation.
