About the role
The Mental Health Accommodation Pathways at Discharge (MHAPD) and Yandina are outreach programs providing support to people who are challenged by mental ill health and who are experiencing, or are at risk of homelessness. The position provides recovery based interventions aimed at successfully supporting clients to address and overcome barriers that contribute to homelessness or increased risk of homelessness. The Community Mental Health Practitioner role is one of direct service provision, predominantly working alongside the client on an individual basis; often within the community.
We are seeking an empathetic, self-motivated Community Mental Health Practitioner for a Full Time, Fixed Term opportunity (until June 2026) to work alongside a dynamic team in the Eastern suburbs area. Working from home, you will provide outreach support, collaborative support and advocacy to clients experiencing homelessness to help them on their journey to achieve their recovery goals, access suitable accommodation, and overcome the barriers that contribute to homelessness. As this position requires travel across the Melbourne Inner East area, a fully maintained company car is provided.
Key responsibilities
- Deliver high quality outreach services and facilitate positive educational and social experiences.
- Support clients to identify and access sustainable housing options and to access services that will help clients to overcome barriers to sustaining and maintaining housing and supports ongoing mental health and wellbeing.
- Complete assessments and documentation that supports the client journey, provide information, and facilitate access to relevant services.
- Work as part of an established team.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.
What you’ll bring
- Relevant qualifications valued – You’ll bring tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, Community Services, or another related field recognised by Mind.
- Expertise in working with people with mental health issues, homelessness issues, complex needs and interacting with families and carers.
- Proven capability to implement recovery oriented frameworks and practices including risk management, trauma informed care and family inclusive practice.
- Proven capability to provide advocacy, collaborative care, goal planning for clients experiencing mental ill health.
- Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
