About the role
We are seeking an independent, self-motivated and compassionate Community Mental Health Practitioner to join the Individual Recovery Support Program (IRSP) in Cairns. IRSP is an outreach service supporting people aged 18 and over who are experiencing mental health and wellbeing challenges to live independently and well in the community. Support is tailored to each person’s needs and preferences and may be delivered face-to-face, online, or over the phone.
In this role, you will work collaboratively with people to achieve their recovery goals through non-clinical case management, care coordination, one-on-one holistic supports, shared supports, and group workshop facilitation. As the position requires travel, a fleet vehicle is provided for business use.
Key responsibilities
- Work with clients to return to a place of residence and a meaningful life supporting them through a staged approach.
- Plan and deliver group programs that build skills, support recovery, and strengthen community connection.
- Facilitate the re-engagement and strengthening of family and carer relationships where appropriate.
- Make linkages and build relationships and referral pathways to maintain or create a range of local supports for clients that facilitate them living the life of their choosing in their own community.
What you’ll bring
- Relevant qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
- Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
- Ability to work both autonomously and collaboratively showing initiative and flexibility.
- Demonstrated understanding of available community services, networks and supports.
- High level organisational skills with demonstrated ability to prioritise, manage multiple complex tasks concurrently, work under pressure and meet deadlines.
