About the role
We are seeking compassionate, self-motivated Community Mental Health Practitioners for casual positions with our Transitional Recovery service based in Rockhampton Region and Yeppoon. In this role, you’ll provide 1:1, shared, and group supports tailored to individual recovery goals. You’ll support residents to:
- Build daily living and self-management skills
- Strengthen social and community connections
- Gain confidence and independence to live well
About the service
The service is based in Rockhampton Region and Yeppoon across 2 houses and is designed to support adults to transition from an acute mental health setting to living in the community and to prevent further hospital admissions.
Clients in the service can access staff 24 hours a day within a home like setting, whilst being supported to achieve their recovery goals, strengthen relationships with families and carers, develop social connections and community networks, build confidence and increase capacity to live safely in the community. The service provides personalised one-to-one support, shared supports, workshops and group activities based on client’s strengths and goals as identified in their individual recovery plan.
Key responsibilities
- Support clients to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery.
- Deliver high quality services and facilitate positive educational and social experiences.
- Work as part of a multidisciplinary team offering specialised psychosocial support.
- Plan and deliver group programs that aid clients to build their skills and focus on recovery.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Youth Work, Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field as designated by Mind.
- Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
- Experience working with people with mental health challenges and complex needs, including families and carers.
- Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
- Current First Aid and CPR certificates.
