About the Service
The Youth Residential Rehabilitation Service (YRRS) provides community-based residential care and on-site psychosocial recovery and mental health support for young people aged 16–21 years. The service offers up to 12 months of longer-term support to assist young people to build the skills and confidence needed to successfully transition to independent living, return to the family home, or move into other suitable accommodation options. Support is delivered within a 24-hour, home-like residential environment.
YRRS works with young people experiencing complex mental health challenges that impact their capacity to live independently in the community. The service is designed for those who are likely to benefit from an extended community rehabilitation model of care that promotes recovery, stability, and independence.
Delivered through an integrated and collaborative partnership with the Hospital and Health Service (HHS), YRRS combines clinical treatment with psychosocial rehabilitation. This model provides a specialised service option for young people whose recovery is best supported through longer-term mental health care within a rehabilitative, residential setting in the community.
About the Role
We are seeking a reliable and client-centered Community Mental Health Practitioner for a casual opportunity in the Townsville area. You will work collaboratively with clients to achieve their recovery goals through one-to-one support, shared supports, workshops and group work facilitation to enhance recovery, personal growth and activities of daily living.
Successful candidates must be available to work a rotating 24/7 roster which includes days, evenings, sleepovers, active nights and weekends.
Key responsibilities
- Work with clients to return to a place of residence and a meaningful life supporting them through a staged approach to recovery
- Plan and develop group work programs that will assist clients to build their skills, focus on their recovery and work towards transitioning back to their natural community.
- Support family and carer roles through understanding their concerns and the provision of information, education and referrals.
- Work with families and carers at the time of transition back to community.
- Work within a multidisciplinary team enhancing collaboration between team members
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
- Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
- Proven experience in Youth Work, Community Services, Mental Health, Disability, Social Welfare, Housing or Healthcare or Government sectors is required.
- Experience in assessing need and working collaboratively to plan goal focused recovery using evidence informed approaches and tools.
- Ability to work both autonomously and collaboratively showing initiative and flexibility.
