About the role
We are seeking a compassionate and self-motivated Community Mental Health Practitioner who is passionate about making a positive impact in the lives of people experiencing mental health challenges. This fixed-term, full-time outreach role offers an exciting opportunity to provide support in clients’ homes and the community, helping them build skills, strengthen connections, and increase independence.
In this role, you will work with clients to achieve their recovery goals, build confidence and skills for safe community living, enhance social connections, and support overall wellbeing. As the position involves travel across the community, a fleet vehicle will be provided.
About the Service
Mind’s Outreach Psychosocial Services South Australia provides person-centred support to help people improve wellbeing, build independence, and achieve their personal goals. The service delivers support through a two-tiered model:
- Intensive Crisis Support (Tier 1): Short-term, flexible support to reduce distress, improve safety, and connect people with longer-term community services.
- Non-Acute Psychosocial Support (Tier 2): Moderate-intensity support over several months to enhance recovery, develop daily living skills, maintain housing, strengthen relationships, build social connections, and support transition to NDIS services.
Key responsibilities
- Deliver high quality outreach services, case managed responses and facilitate positive educational and social experiences.
- Plan and deliver group programs that aid clients to build their skills and focus on recovery.
- Support clients with strengths based, recovery-orientated approaches to develop daily living skills, capacity and resilience.
- Provide information and mentoring to clients to establish connections with community, families and carers, create networks and access to relevant services.
- Collaborate with a range of services, families and the community to ensure a co-ordinated, integrated response to client’s recovery goals.
What you’ll bring
- Relevant qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
- Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
- Experience applying therapeutic approaches such as trauma-informed care, cognitive behavioural therapy, motivational interviewing, harm reduction, and family-inclusive practice.
- Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
