About the role

The Mental Health Integrated Complex Care (MHICC) service, funded by the South Eastern Melbourne Primary Health Network, provides specialised supports to clients with severe mental ill health and complex needs who will benefit from improved care coordination over an extended period of time. MHICC supports people that are harder to reach, underserviced or cannot access similar supports under the NDIS and delivers highly flexible outreach services tailored to meet individual needs of clients. Services include clinical nursing services, care coordination and liaison with service providers, family support and improved access to psychiatry and psychological care.

We are seeking a compassionate and experienced Team Leaders for a Permanent, Part-time (30.4 hours per week) opportunity to work in the Dandenong area. You will provide leadership and support to ensure the delivery of high-quality recovery-oriented services to clients, families and carers in line with Mind’s Model of Recovery Oriented Practice, My Better Life model and organisational values. The Team Leader role is integral in being a practice led to Credentialed Mental Health Nurses and Community Mental Health Practitioners providing supervision, mentoring and coaching. The role will liaise with a number of providers both internally and within the community, and will have delegated authority to make day to day decisions to ensure the operational requirements of the service are met.

 

Key responsibilities

  • Lead delivery of services to clients, carers and families as defined in the Model of Recovery Oriented Practice and related guidelines to ensure that individually tailored services for clients and their family and carers are provided to the highest standard.
  • Oversee written tasks including case note writing, risk assessments, safety plans, behaviour management plans and incident reports.
  • Lead, coach and model a targeted, integrated and individualised responses to clients through flexible, person-centred active support.
  • Engage with clients to fully understand their need for assistance from local service providers with clinical services, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation and social connections.
  • Work within a multidisciplinary team in supporting a recovery-oriented practice, clinical interventions and participate in team, case and handover meetings.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing, Healthcare or Government sectors is required.
  • Demonstrated knowledge and experience in the application of theoretical approaches, practices and appropriate service responses including family inclusive practice, trauma informed theory, cognitive behavioural therapy, motivational interviewing and harm reduction therapy.
  • Demonstrated understanding of available community services, networks and supports.
  • Proven experience in leadership and capability to coach, mentor and develop a team to achieve best quality service outcomes including experience in the provision of professional supervision
