About the role

We’re looking for a compassionate and motivated Lead Practitioner – Family Engagement to join our Adult Step-Up Step-Down residential service in Bundaberg on a fixed-term, part-time basis.

 

In this role, you’ll provide practice leadership and work collaboratively with clients, families, carers and staff to support positive recovery outcomes. You’ll help ensure families and carers feel welcomed, supported and meaningfully involved, while providing information, advice and referrals and working closely with clinical and community partners.

 

You’ll also coach and mentor Community Mental Health Practitioners and support the delivery of high-quality, recovery-oriented psychosocial rehabilitation and support services.

 

About the Service

The Adult Step-Up Step-Down (ASUSD) is a short-term (up to 28 days) 24/7 residential service delivered in partnership with Queensland hospital networks and the local health service. It supports adults with a psychosocial disability to either transition from an acute hospital setting or prevent hospital admissions.

 

Key responsibilities

  • Provide direct support to clients through recovery planning, skill development and family-inclusive practice.
  • Support families and carers through education, advocacy, engagement and participation in recovery planning.
  • Coach, mentor and support Community Mental Health Practitioners to build capability and promote high-quality recovery-oriented practice.
  • Plan, facilitate and evaluate group programs focused on recovery, wellbeing and community participation.
  • Develop and maintain partnerships with clinical teams, community services and other stakeholders to support referrals, service linkages and coordinated recovery outcomes.
  • Lead family engagement initiatives and contribute to continuous practice improvement across the service.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Demonstrated experience working with people experiencing mental ill health, complex needs, and their families and carers.
  • Experience in family engagement, family-inclusive practice and supporting carers.
  • Proven ability to coach, mentor and support practitioners to deliver high-quality recovery-focused services.
  • Current First Aid and CPR certification.
  • Ability to provide proof of vaccination or willingness to obtain required vaccinations in line with Queensland Health partnership requirements.
