About the Service
The Bilingual Hospital 2 Home program helps people from culturally and linguistically diverse communities living in Western Sydney to connect to mental health services, develop resilience, increase understanding around mental health and reduce hospital readmission.
The program supports people living with mental health challenges including people upon discharge from hospital, It provides integration into the community by facilitating community-based support groups, delivering person-centred, recovery-oriented, family focused and trauma informed services.
About the role
We are seeking a collaborative and culturally competent Bilingual Mental Health Worker for a 12 month fixed term, part time (30.4 hours per week) opportunity in the Parramatta area. You will build relationships and capacity to strengthen co-ordination, referral pathways and connections between GPs, Allied Health Providers, Government Agencies, Health Services and Community Services to improve support for individuals from culturally and linguistically diverse populations who are impacted by severe and persistent mental health conditions.
Key responsibilities
- Work with the people we support in the mental health units to help plan their discharge with the mental health team, define their goals and needs for the first 6 – 12 weeks after discharge, and then support the individual in the process of implementing the plan and linking to relevant services and supports.
- Build collaborative relationships with key culturally and linguistically diverse community leaders and members to support individuals, families, and carers in those communities to improve access and connections to appropriate support services.
- Provide Community and Consumer lifestyle, capacity building, and education Groups and promote weekly social connection and ward for consumers in the program.
- As part of the recovery 2gether team, you will work collaboratively with other team members to facilitate the effective operation of the team.

What you’ll bring
- Relevant tertiary qualifications to Certificate IV in mental health, peer work or related field, and/or two years of experience in mental health related work.
- Bilingual – fluent in Cantonese and/or Mandarin to ensure culturally and linguistically appropriate care
- An understanding of the recovery- based approach to assisting people with severe and persistent mental illness
- Knowledge of the NSW mental health system and local services
- Ability to communicate effectively in all interactions with all stakeholders
