About the role
We are seeking a compassionate Community Mental Health Practitioner to support young people (12–25) experiencing, or recently recovered from, a first episode of psychosis. You will provide primarily outreach support across Metro South Brisbane, with flexibility to work from home as needed. You’ll help young people achieve recovery goals, build independence, and engage in education, employment, and community life.
Working closely with families, carers, and clinical teams, you will deliver one-on-one and group support, coordinate care, and provide guidance and referrals, making a real difference in the lives of young people and their communities.
Key responsibilities
- Support young people to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery
- Engage clients using brief interventions, motivational interviewing, trauma-informed care, and family-inclusive practice
- Plan and facilitate group programs to build young people’s skills, confidence, and community connections
- Support families and carers with education, information, and referrals
- Collaborate with local service providers to connect young people with mental health, physical health, education, employment, housing, and social connections
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
- Experience supporting young people with complex mental health needs, ideally in outreach or community settings.
- Ability to work both independently and collaboratively, demonstrating initiative and flexibility.
- Current, unrestricted driver’s licence and access to a registered roadworthy vehicle.
- Ability to provide proof of vaccination against vaccine preventable diseases or willingness to obtain required vaccinations per Queensland Health partnership.
