About the role
We are looking for self-motivated and compassionate Community Mental Health Practitioners to join our Caboolture team and work across two youth residential services.
In this role, you will provide recovery-oriented, person-centred support to young people aged 16–21, helping them to:
- Achieve their recovery goals
- Build confidence and skills for independent living
- Develop daily routines and independent living skills
- Strengthen social and community connections
- Develop resilience and maintain wellbeing
You will work collaboratively within a multidisciplinary team across a 24/7 rotating roster, including weekends, overnight shifts and sleepovers.
About the services
Youth Residential Rehabilitation Service (YRRS)
A longer-term residential program providing a safe, home-like environment where young people can build independence, resilience, life skills and social connections while working towards their recovery goals.
Youth Step Up Step Down (YSUSD)
A short-term residential program delivered in partnership with the local health service, offering a supportive therapeutic alternative to hospital admission "step up" or assisting young people transitioning from hospital back into the community "step down".
Key responsibilities
- Support young people to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery
- Purposefully engage with young people using techniques including: Brief intervention, Motivational interviewing and coaching, Family inclusive practice, Trauma informed practice, Conflict resolution
- Plan and deliver group programs that aid young people to build their skills and focus on recovery
- Support family and carer roles through understanding their concerns and the provision of information, education and referrals
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to people’s recovery goals
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Youth Work, Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field
- Expertise in working with young people with mental health issues, complex needs and interacting with families and carers
- Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice
- Hold a current First Aid and CPR certification
- Current Blue card and NDIS Workers Screening Check (yellow card), or willingness to obtain
- Hold a valid Australian, Driver’s licence
- Ability to provide evidence of required vaccinations in accordance with Queensland Health partnership requirements
