About the Service

The Adult Prevention and Recovery Care (PARC) service is a sub-acute, short-stay residential program delivered in partnership with Mind and local health services to support adults aged 25-65 years experiencing mental ill health. Designed as a ‘step up, step down’ service, Adult PARC provides an alternative to hospital admission for people requiring additional support, as well as assisting those transitioning from an acute inpatient setting back into the community.

 

About the role

We are seeking a skilled, collaborative and inclusive Lead Practitioner – Family Engagement Worker for a permanent. part time (30.4 hours per week) opportunity in the Heidelberg Heights. You will work closely with participants, families, carers and clinical partners to deliver integrated, recovery-focused support. This includes providing information, facilitating referrals and ensuring services are responsive to individual needs and goals.

 

As a practice leader, you will provide guidance, mentoring and direction to staff delivering psychosocial rehabilitation and support services, fostering best practice and ensuring the delivery of high-quality, person-centred care.

 

Key responsibilities

  • Coach and mentor Community Mental Health Practitioners to provide quality support to clients
  • Create, facilitate, evaluate and review professional development training sessions which reflect the needs of the teams at a local level.
  • Advocate for families and carers in supporting for an effective coordinated transition from residential care back into the community.
  • Work with the team to identify and overcome barriers to engagement/motivation in families and carers.
  • Support family, existing support networks and significant others to assist client through treatment using family inclusive interventions.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
  • Ability to co-design, co-produce and co-facilitate groups and education support.
  • Proven capability to coach, mentor and develop a team to achieve best quality service outcomes including experience in the provision of professional supervision.
  • Skills and experience in advocacy, collaboration, facilitation, evaluation and problem solving.
