About the role

The Community Recovery Program (CRP) service, in conjunction with the North East Area Mental Health Service, provides 24/7 residential psychosocial rehabilitation for clients with psychiatric disability. Recovery-focused support, treatment and care is provided to clients moving directly from the community or stepping down from a psychiatric in-patient unit, secure extended care unit or Forensicare. The CRP service enables individuals to achieve recovery outcomes by developing skills, enhancing social relationships, building community connections and collaborating with clinical mental health services, service providers, agencies, families and carers.

We are a compassionate, self-motivated Community Mental Health Practitioner for a Permanent, Part Time (30.4 hours per week) opportunity to work alongside a caring team in the Heidelberg area. With 24/7 rotating roster + active night shifts, the Part-Time renumeration + penalty rates of these positions will be comparable to Full-Time earnings! In the role you will provide intensive person-centred, recovery-oriented supports to clients to achieve their wellbeing goals, build capacity and daily living skills, strengthen social and community connections, and live independently in the community.

 

Key responsibilities

  • Plan and develop group work programs that will assist clients to build their skills, focus on their recovery and work towards transitioning back to their natural community.
  • Engage with clients to fully understand their need for assistance from local service providers with clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation and social connections.
  • Work with clients to return to a place of residence and a meaningful life supporting them through a staged approach to recovery.
  • Purposefully engage with clients using techniques including brief interventions, motivations interviewing and coaching, family inclusive practices and conflict resolution.

 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing or Healthcare or Government sectors is required.
  • Experience and expertise in working directly with people with mental health issues, homelessness issues, complex needs and with their families and carers.
  • Demonstrated knowledge and experience in the application of theoretical approaches, practices and appropriate service responses including family inclusive practice, trauma informed theory, cognitive behavioural therapy, motivational interviewing and harm reduction therapy.
  • Excellent interpersonal and communication skills with the ability to consult, negotiate and influence peers, stakeholders and government agencies with diplomacy to achieve effective outcomes.
